New billing G Codes for registered nurses

On January 1, 2016, Change Request 9369 goes into effect, requiring that home health and hospice agencies use two separate G Codes when billing for skilled registered nurse (G0299) and licensed practical nurse services (G0300) to distinguish which service was provided during each visit.

The reason for this change is to support compliance with the Service Intensity Add-On (SIA) payment for skilled hospice visits by a registered nurse and/or medical social worker provided during last seven days of life during a hospice election. Visits by licensed practical nurses do not qualify the hospice agency for the SIA payment; therefore, simply billing for skilled nursing services, as was done with G0154, would not provide the necessary information to determine whether the SIA add-on applies.

Although the National Association for Home Care and Hospice is pushing to delay implementation of this requirement due to time limitations in preparing billing software, the change request at this time is scheduled to be implemented for hospice dates of service on and after January 1, 2016 and for home health episodes of care ending on or after January 1, 2016.
